This statement explains our privacy practices for both our website and our clinical services in accordance with the Personal Health Information Protection Act (PHIPA) and other applicable privacy legislation.

We collect information to provide high-quality therapeutic services and manage our practice.
Personal Information: Through our website or intake, we may collect names, email addresses, phone numbers, and other contact details.
Personal Health Information (PHI): PHI is any information that can identify you and relates to your health, treatment, or payment. This includes health history, diagnosis, treatment plans, progress notes, risk assessments, billing records, and therapy attendance.
Automatically Collected Information: When visiting our website, we may collect data (IP address, browser type, pages visited) using cookies to improve website functionality and user experience.
2. Our Role as Health Information Custodian (HIC)
Saero Therapy is the Health Information Custodian (HIC) responsible for your PHI.
Administrative Oversight: While your therapist is your primary provider, our Privacy Officer and authorized administrative staff may access your file only when strictly necessary to perform essential administrative duties.
Purpose of Access: Administrative access is restricted to the minimum information required for maintaining record accuracy, facilitating your access requests, or ensuring our security standards comply with PHIPA.
3. How We Use and Share Information
We use your information only for legitimate purposes:
Clinical Care: Understanding your needs, providing therapy, and monitoring progress.
Care Coordination: Sharing PHI with other providers only with your express consent.
Operations: Managing billing, insurance claims, and scheduling.
Legal Compliance: Complying with professional college requirements or legal orders.
We do not sell, rent, or trade your information.
4. Safeguards and Retention
Security: We implement administrative, physical, and technical safeguards (such as password protection, encryption, and secure storage) to protect your data.
Retention: We retain clinical records for at least 10 years after the last appointment (or 10 years after a minor turns 18), as required by professional standards.
5. Your Rights
Under PHIPA, you have the right to:
Access: Request a copy of your records.
Correction: Request amendments to inaccurate or incomplete information.
Complaints: Raise concerns if you believe your information has been mishandled.
Right to Review: Please note that your right of access is subject to limited exceptions under the law. Before records are released, they undergo a mandatory clinical review to ensure compliance with PHIPA, which may include redacting information that involves third-party confidentiality or withholding records if access could reasonably be expected to result in a risk of serious harm to yourself or others.
6. Privacy Breaches
A privacy breach occurs if PHI is lost, stolen, or accessed by unauthorized persons. If a breach occurs that poses a risk of harm, we will notify affected individuals and, where required, the Information and Privacy Commissioner of Ontario (IPC).
